 ______    ______ 5.   I have filled out a "Troop Camping", form , three weeks in advance of the trip, mailed it to the Service Unit Manager or Troop Coordinator and received her approval.

 ______    ______  6.   I have received a signed "Parent Permission" Slip from each child, and have a                                              completed Health Statement for each person going camping.  I have                                                   provided the names of the children going on the trip to the emergency contact
                                    person.

 ______    ______ 7.   I have arranged for a currently qualified Red Cross First Aider, Nurse to                                                                 accompany the group.

 TROOP/GROUP

 ______    ______ 1.   The group has learned to PLAN AHEAD for the group camp over a period of at                                                    least four troop meetings, prior to the trip.
